St George's Confidential Admission Form

Car Parking

A Car Park Building is available for all visitors on the site.  This is open Mondays to Fridays only.  A charge of $1.10 per every half hour or part thereof applies.  There is NO CHARGE if you exit the site within 30 minutes.

Admission

Your surgeon or physician will make the necessary arrangements for your admission to St George's Hospital and advise what time to arrive at the Hospital. He or she will give you an admission form to complete. Please submit your admission form to the Hospital at LEAST 3 DAYS PRIOR TO ADMISSION.

Please notify the Hospital at your earliest possible convenience if you have a latex allergy or any special dietary requests.

Please follow your surgeon's instructions about eating prior to surgery. If, for example your surgeon states "nil by mouth from 8.00 am" it is very important that you do not eat or drink after 8.00 am on the day of your operation.

It is usual for the anaesthetist to see you in Hospital prior to surgery. Your operation may be scheduled towards the end of the operating list. This will result in delay between your admission and your operating time. We suggest that you bring in reading material or something to do to help pass the time.

What to Bring - Valuables and Money

You will require:

  • Any medication and drugs in original packets or inhalers that you are currently (or have recently) been taking
  • Any X-Rays (which may be relevant), crutches or braces
  • Details of your medical insurance (if applicable) including copies of any letters of authorisation and your membership number
  • Supply of sleepwear, dressing gown, slippers
  • Personal toiletries
  • Glasses, lenses, hearing aids.

Children should bring pyjamas / nightdress and a favourite toy.

We prefer that valuables and money are kept to a minimum and whenever possible left at home. This Hospital cannot accept responsibility for any items. If you have any valuables, please inform staff as safety deposit facilities are available.

Parents with Children Having Surgery

Many parents wish to stay with their children while they are in Hospital; and they are encouraged to do so.

Most children's hospital stays are short and each child's requirements are unique. We encourage parents to discuss their child's accommodation arrangements with their surgeon prior to admission. Once in Hospital please do not hesitate to discuss your child's needs with the Charge Nurse.

Meals can be purchased by parents in the Hospital Café. There is no charge for a parent wishing to stay the night in a lazy boy chair in the child’s room, however if you wish to stay the night in a bed and have a full meal service you will be charged a fee. Parents may also accompany children to the Anaesthetic Room if they wish.

Partner - Relative Staying Overnight

In rare and exceptional circumstances it may be appropriate for a relative / partner to stay overnight. This needs to be arranged with the Charge Nurse prior to admission if possible.

Meals

Please state special dietary requests on your admission form.

The DeliMarché Café is available for visitors and relatives to use. This is located on the ground floor, Canon Wilford Wing.

There is a snack vending machine located in the Atrium, between the Canon Wilford Wing and The Heritage Building.

Discharge

Your surgeon will discuss your discharge date and instructions with you. You will also receive written information about pain relief, wound care, blood clots and a follow-up appointment time with your surgeon. At the time of your discharge if you do not fully understand anything about your operation, illness, subsequent care or have any concerns about your discharge, please do not hesitate to ask your Charge Nurse or doctor.

In preparation for your discharge from the ward please make sure you have:

  • Instructions about what to do after your operation
  • Your medications and/or prescription
  • An appointment to see your surgeon
  • A responsible adult to accompany you home and someone who will stay with you for the first night after your operation / procedure (patients undergoing cardiac surgery need to discuss the additional requirements with staff)

Please arrange for your discharge to be between 9.00 am and 10.00 am as this gives staff sufficient time to prepare rooms for new admissions. The Hospital reserves the right to charge a late discharge fee.

Prior to leaving the ward, please check with the nursing staff. If you are feeling unwell please discuss your discharge with your Charge Nurse.
