Your Clinical Record
Your Clinical Record is held in the Clinical Records Department at St George's Hospital for a period of approximately 15 years from the date of your last attendance.
In most instances your Surgeon will forward information to your GP but we are able to forward information to other health practitioners or health care facilities upon your request.
If you require information yourself, " Health Information Request" forms are available from the Hospital’s Main Reception, can be posted to you by Clinical Records staff, or (downloadable here in PDF format or Microsoft Word Document format) and can be e-mailed to Clinical Records. They require personal identification and a description of the information you require.
